What is Home Health Care?

When patients are recovering from hospitalization, surgery or a major health event, Home Health Care provides nursing and therapy services in patients’ homes focused on rehabilitating, supporting recovery and managing chronic illnesses.

 

When is Home Health Care Appropriate?

Home Health Care must be ordered by a physician when a patient is homebound and has a medical necessity for skilled care. 

 

 

Homebound is when:

  • It is recommended you stay home due to your health condition
  • It takes a considerable amount of effort for you to leave your home
  • You can’t leave your home without assistance from another person, special transportation or equipment

Examples of Skilled Care:

  • Intermittent skilled nursing care
  • Physical therapy
  • Speech-language pathology services
  • Occupational therapy

  • Clinical Supervisors: Responsible for the oversight of all care you receive and ensure the best practice standards are followed
  • Registered Nurses: Coordinate your plan of care and ensure individual needs are met
  • Therapists: Physical therapists, occupational therapists and speech-language pathologists address safety needs. They provide rehabilitative services after surgery or acute illness, or during an exacerbation of a chronic condition
  • Medical Social Workers: Assist you and your family in coping with stress, emotions and challenges related to your illness as needed
  • Home Health Aides: Assist with bathing, personal hygiene and general activities of daily living
